MAILING APPLICATION INSTRUCTIONS

1.

Enter names as they will receive mail. Include any nicknames, spouse names, maiden names, and minor children.

2.

List any Associate Members - parents, adult children, siblings, company name(s), etc.

3.

Include your current address, even if you are "moving" to our address. We need this information for historical purposes

4.

List the date that you wish to begin service, the length of service, and the frequency of mailing desired. (Be sure to list "Special Instructions" if you have any)

5.

If you already have a forwarding address please list it. If not, you may call us or send an email when you get, or change, an address

6.

List any phone numbers where we will be able to reach you

7.

List your email address (you will not be able to apply online without entering an email address)

8.

Enter Emergency Contact information, if applicable
